Permission to Publish Student Work on the Web

Sample letter to parents:

March 17, 2003

Dear parent/legal guardians,

For the purpose of sharing with the community the exemplary work of your child, the School District of Black River Falls is asking permission for your child’s work to be published for a reasonable period of time. Please complete the form below and return it to the identified teacher.

Please return this portion to __________________________ (teacher’s name), as instructor of the _______________________________ course for which the work was developed.

______ I give permission for my child to have his/her work published on the Black River Falls SD website. I hereby release the School District of Black River Falls from any and all liability and legal or equitable claims of any kind related to such work being published on the district web site. I understand that the student work will be identified by first and last name.
